(1) In the absence of other provisions, there is always a way to apply to
register/allocate any name/number provided for in any IETF standard.  Contact
IANA.  (This doens't mean you will necessarily get what you want from IANA.)

(2) Your statement above is almost right except that these *were* ISO 
3166 TLDs, weren't they?
